  • Changes include treatment plans for physical and mental health.

  • First Psychiatrist appointment booked in two years. 

  • Mental Health and Crisis Support.

  • Involvement in two events when she wanted to take her own life and follow-up support, including weekends and evenings. 

  • Introduced to crisis text support (Shout). 

  • Reintroduce to embroidery as a coping strategy. 

  • Daily long phone calls with her seven days a week.

Medical Health and Support

  • Practical help with heavy rubbish bags. 

  • Finding space for her work area so she can start to write a book. 

  • Gardening and outdoor maintenance.

  • Front and back gardens cleared and tidied. 

Home and Safety Improvements 

Nutritional and Meal support

  • Organised meals on wheels 7 days a week. 

  • Eating new foods; less reliance on build-up drinks.

Legal and Financial Support

  • Benefits and  financial management: 

  • Benefits (PIP) confirmed in place. 

  • Spending cut by 75% each month (mostly clothes). 

  • Support with PayPal problems and possible fraudulent activity. 

  • Council tax exemption agreed. 

  • Helping her remove her mother’s name from bank accounts. 

  • Stop water company from cutting off; debt collection agreed to wait. 

  • Support with Barrister and Solicitor; regular contact with Solicitor to steer the process in the right direction, avoiding crisis scenarios.

  • Cancellation of Home Care as ineffective. 

  • Hygiene and self care monitoring.

  • First bath in at least eight years, now weekly. 

Community Engagement

  • Social Interaction and activities to increase in the community. 

  • Planning to get her to church. 

  • Birthday party in a pub. 

  • Will not be alone this Christmas for the first time in years.

  • PA now shopping with her to choose her own food. 

  • Accompanied her to Co-op to apologize for past behavior and theft of goods (goods paid for and chocolates given to staff).

  • Register cat as a therapy pet. 

  • Improve cat care (De-flea, regular emptying of tray).
